The Clinical Practice Guidelines are confidential and proprietary. They are to be used only as authorized by Plan My Health Services Limited and its affiliates. The contents of these Clinical Practice Guidelines are not to be copied, reproduced, or circulated to other parties without the express written consent of Jainam Health. The contents of these Clinical Practice Guidelines may be updated or changed without notice. However, benefit determinations are made in the context of Clinical Practice Guidelines existing at the time of the decision, and are not subject to later revision as the result of a change in Guidelines.
The purpose of the Clinical Practice Guidelines database is to provide Plan My Health Services Limited with a tool for the objective comparison of clinical office practices among network participating physicians. These guidelines are not intended to direct the course of clinical care you provide to an individual Plan My Health member. Neither do these guidelines replace your independent professional clinical judgment or your professional duty to exercise your special knowledge and skill in the treatment of your patients. As set forth in your participating provider agreement, you remain responsible for the quality and type of health care services provided to Plan My Health members.
These guidelines were adopted from nationally known organizations such as the HEDIS (Health Plan Employer Data Information Set) initiatives and the different wellness and disease management programs we offer our members.
